Maternal health in rural India has undergone a radical change over the last two decades. The government's maternal health initiatives (e.g., Janani Suraksha Yojana (JSY), Janani Shishu Suraksha Karyakram (JSSK), Pradhan Mantri Matru Vandana Yojana (PMMVY) and Pradhan Mantri Surakshit Matritva Abhiyan (PMSMA)) have transformed the institutional delivery landscape, upgraded antenatal care and reduced costs to care. At the same time, community-based systems under the leadership of Accredited Social Health Activists (ASHAs) and traditional care practices based on Ayurveda and support at home remain part of the maternal experience. This paper contrasts the impact of these government interventions with conventional systems of care based on National Family Health Survey (NFHS) data, Sample Registration System (SRS) bulletin and ethnographic evidence on maternal care in rural areas. Findings indicate that although institutional programs have achieved significant success in reducing maternal mortality and in enhancing the utilization of services, traditional practices continue to offer an essential cultural sanction and postpartum care. Blended approach needed to fill gaps in access, quality and cultural appropriateness.
